Departmental Deposit Request For departments wishing to request a Print & Copy Card or to deposit Plaid Cash to a student account. 6/2016
Plaid Cash Withdrawal Request For students separating from the university who have a Plaid Cash balance of $50 or more. 6/2016

  • If you wish to complete your form electronically, you must first save it locally and then type in your information before submitting.
  • Submit your completed form with appropriate signatures to the address, fax or email listed on the form.
  • At this time, we do not accept electronic signatures - so if a signature is required, please print and sign the form.
